Volunteer Application Form

Silver Care Dementia Center, INC provides basic companion care, respite care, education, and training programs to those living with dementia, their caretakers, and the community. Due to the harsh realities of no affordable home care and minimum resources, have deprived most caretakers of people living with dementia the self-care they need.

We encourage the participation of volunteers who support our mission and are willing to contribute. The information provided through this form will be kept confidential and will help us determine the most satisfying and appropriate volunteer opportunity for you.

Liability Release:

As a volunteer of Silver Care Dementia Center, INC. I agree to abide by all policies and procedures as spelled out in the volunteer handbook. I understand that I volunteer at my own risk and neither the organization nor its employees assume any liability for any accidental injury or health problem arising from volunteer work I perform for the organization. I agree that all work I do is on a volunteer basis, and I am not eligible to receive any monetary payment or reward.
